  • Walker trial, if required

  • Completion and submission of Assistive Devices Program (ADP) application to your chosen vendor (if eligible)

  • One in-person visit

  • Location: Your home or a vendor location

Power mobility
Power Chair Assessments

What to expect:

  • Initial interview to determine appropriate service and equipment needs

  • Comprehensive physical, cognitive, and functional assessment

  • Completion and submission of Assistive Devices Program (ADP) application to your chosen vendor (if eligible)

  • Power Mobility Indoor Driving Assessment (PIDA)

  • Follow-up visit after equipment delivery to ensure optimal fit and function

  • Typically requires 3 visits

  • Location: Your home or hospital

Manual Wheelchair and Scooter Assessments

What to expect:

  • Initial interview to determine appropriate service and equipment needs

  • Comprehensive physical, cognitive, and functional assessment

  • Equipment trials, if required

  • Completion and submission of Assistive Devices Program (ADP) application to your chosen vendor (if eligible)

  • Follow-up visit after equipment delivery to ensure optimal fit and function

  • Typically requires 2 visits; additional visits may be required and are billed accordingly

  • Location: Your home or hospital

  • Please note: For ADP eligibility, power devices must be required for use within the home. Scooters used for community mobility only are not eligible for funding.

  • Seating equipment trials, if required

  • Typically requires 2 visits; additional visits may be required and are billed accordingly

  • Location: Your home
